The document is deleted within minutes, but the AI agent has already ingested it, and it’s now part of its internal memory of the model. No one intended for that to happen. Worse, many operate in silos with no audit logs or behavioral telemetry. The incident stemmed from an exposed Elasticsearch database containing protected health information (PHI) accessed by backend systems operated by AI agents, without triggering any traditional security alerts. The data exposure was only discovered later during an external audit. This underscores the core risk: AI agents can access and act on regulated data without context or intent, bypassing legacy DLP and SIEM tools.</span><span data-ccp-props="{}"> </span></p><p><span data-contrast="auto">It is critical to have the ability to maintain security controls, audit trails and remove sensitive data from the memory of the AI models, essentially unlearning material when it violates policy.</span><span data-ccp-props='{"335559738":240,"335559739":240}'> </span></p><h3 aria-level="3"><b><span data-contrast="none">Traditional Controls Aren’t Built for Agents</span></b><span data-ccp-props='{"134245418":false,"134245529":false,"201341983":0,"335559738":280,"335559739":80,"335559740":297}'> </span></h3><p><span data-contrast="auto">Role-based access control (RBAC) was designed for humans in well-defined job functions. But agentic access is probabilistic. </span><span data-contrast="auto">Agents might ingest hundreds of documents just to answer a simple query. Agentic AI introduces systems that act autonomously and interact fluidly with other agents, users and systems. This evolution requires a security architecture built on intelligent coordination.
